    Default Alarm went off... silently

So I set up an alarm to wake me today, made it periodical (every monday, wednesday and friday) and after getting a fair amount of sleep I wake up to an amazing discovery of being late. While I blamed myself of not ticking it at the list of alarms, which already happened to me once or twice, when I picked up my phone it turned out that the small alarm window showed, but with no sound or vibration at all. Trying to adjust the volume didn't make it sound and, strangely, didn't close the window at once. The alarm apparently went off, but came in silent like a ninja instead of being loud like an atomic war.

I am not using any task killing apps. The only app I installed that is continuously running is Handcent SMS. I have my alarm set to ring even in silent mode. Everything is up do date. Of course, I wasn't able to recreate the problem, and I tried a fair amount of times (amongst other options, trying silent mode and vibrations mode), always making it ring.

Why this happened? How can I be absolutely sure, that no matter what, my phone will wake me up?

Side question: what determines whether this small window with "snooze and "turn off" buttons appear or it's a whole screen notification with a huge alarm clock icon, with a control like the one to unlock the phone to turn off the alarm? It happened to me that the small window got lost somehow when I accidentally locked the phone instead of turning off the alarm. Made me set up another alarm just to turn off the "lost" one. It's not a big issue however, because, while being annoying, it wake me up anyway.

    It's a while ago, but:
I have experienced exactly the same these days on my Sensation!
And I don't know what to do about it - have you found any solution?

    Greetings from Denmark!

    Jakob :-)
